PLEASE READ CAREFULLY – By signing this waiver, you acknowledge and accept the following terms and conditions, and agree to release Tiny Beans Play Cafe from any liability associated with the use of our play floor.

Waiver and Release of Liability

I, the undersigned, as the parent or legal guardian of the child(ren) named above, hereby acknowledge and agree to the following:

1. Assumption of Risk: I understand that participation in the activities provided by Tiny Beans Play Cafe involves certain inherent risks, including, but not limited to, risks of injury, falls, accidents, and other activities on the play floor. I acknowledge that it is my responsibility to supervise my child(ren) at all times while using the play floor and facilities.

2. Release of Liability: In consideration for being allowed to use Tiny Beans Play Cafe’s facilities and services, I, on behalf of myself, my child(ren), and any other party related to this visit, hereby release, waive, and discharge Tiny Beans Play Cafe, its employees, agents, and representatives from any and all liability for injuries, damages, losses, or claims arising from or related to my child(ren)’s use of the play floor, including any claims based on negligence or other acts of omission.

3. Medical Authorization: I give permission for Tiny Beans Play Cafe to seek medical treatment for my child(ren) in case of an emergency. I understand that any costs incurred for medical care will be my responsibility.

4. Health and Safety: I agree that my child(ren) will not use the play floor if they are ill or displaying symptoms of illness, and I will notify Tiny Beans Play Cafe immediately if my child(ren) is not feeling well or has any communicable illness.

5. Rules and Conduct: I agree to ensure that my child(ren) follows the posted rules and guidelines of Tiny Beans Play Cafe at all times. I understand that failure to comply with these rules could result in removal from the play floor.

6. Photographs and Videos: I understand that Tiny Beans Play Cafe may take photographs or videos during my visit for promotional purposes. By signing this waiver, I give consent for my child(ren)’s image to be used in such materials. If I do not wish for my child’s image to be used, I will inform staff prior to my visit.

7. Indemnification: I agree to indemnify and hold harmless Tiny Beans Play Cafe, its employees, agents, and representatives from any claims, demands, damages, or costs resulting from my child(ren)’s participation in the activities or use of the play floor.

8. Governing Law: This waiver and release shall be governed by and construed in accordance with the laws of the state of Virginia.
